Dispatch request forms

UniMail has a number of forms that staff may need to fill out.

Ensure all fields are completed on both forms to avoid delays in processing for dispatch.

MDR-04 Local and Interstate Mail and Courier form

This must be completed for all:

• outgoing mail to be sent by local, intrastate or interstate courier
• mail that is to be charged against a business unit/project grant that is different from the default business unit/project grant for the Mail Bag Delivery Point (MBDP).
